Stealth Broadband has been awarded funding through Nebraska's Broadband Equity, Access, and Deployment (BEAD) Program to bring reliable, high-speed fiber Internet to eligible homes and businesses in funded project areas. Applicant Information

This form is to be completed by the adult owner or tenant at the address provided. Note, the Owner, or authorized representative, must sign the Agreement, below, to authorize installation.

Your Responsibilities

Before we install:

• Property Owner must sign the Agreement, below.

• Mark or expose any private underground utilities (e.g., irrigation, private wiring, hidden obstacles).

• We will follow all rules for public utility markings.

• We are not responsible for damage to unmarked private utilities or hidden obstacles.

• If damage occurs you/Owner are responsible for repairs and costs.

Agreement

By signing and dating below, you understand this is a service line installation authorization, waiver, and release of liability form.

• We, Stealth Broadband, and/or its assignees (e.g., Bauer Underground) will request 811 location services on behalf of Property Owner, including the marking of public utilities by appropriate utilities service providers, in accordance with applicable law (including Nebraska Code). No construction digging or work will begin on the fiber optic network project until marking is complete and a positive response or report is received. Stealth Broadband and its assignees will maintain appropriate clearance from markings and will follow all rules pertaining to working near public utilities.

• Your responsibility is to mark any private underground utility lines, service connections, and hidden obstacles on your property – if any. This includes but is not limited to underground wiring and irrigation systems, and private water, sewer, or drainage lines which must be marked or exposed by you, the Property Owner. Stealth Broadband will maintain a safe work distance from those markings.

• Stealth Broadband will not be responsible for any damage to unmarked or inaccurately marked private underground facilities, obstacles, or hazards, including but not limited to rock, cement, water, or sewer lines. If private utilities are not marked or are inaccurately marked, and damage occurs that is noticed during the work, the damage will be flagged and, if practical and safe, left exposed to allow for repairs. Damage will be reported to the Property Owner, and the Property Owner is responsible for making repairs and all costs associated with them.

I, the undersigned Property Owner (or authorized representative of the owner), hereby grant permission for Stealth Broadband and/or its assignees to enter upon my property and install, maintain, and repair a buried broadband service line. I further authorize Stealth Broadband and/or its assignees to request 811 location services on behalf of Property Owner for the purpose of constructing, installing, or repairing the service line. I consent to installation of a service line to my property and release, hold harmless, and agree to indemnify Stealth Broadband, its contractors, and employees from any claims, damages, or costs arising from injury to or damage of underground utilities, facilities, or structures located on the property, including but not limited to any unmarked or inaccurately marked private utilities or hidden obstacles, and agree to be solely responsible for the costs of locating and marking private utilities and for any resulting repairs.
